As a Home and Community Services Project Manager, you will oversee the efficient operation of our domiciliary care service, ensuring quality control, managing client needs, and leading staff to deliver exceptional care.

In this dynamic and engaging role, you will:

  • Collaborate closely with the Care Manager, participating in a structured induction program supported by our dedicated Home Care service delivery team.
  • Network within the local community to increase awareness of our services and uphold the highest standards of care.
  • Recruit, train, and retain staff by adhering to best practices and guidelines.
  • Coordinate with professionals and care organizations to enhance and develop client care packages.
  • Effectively handle all emergency on-call issues, including covering shifts when Care Coordinators are unavailable, and ensure proper documentation and communication of these emergencies.

  • Ensure fair and appropriate allocation of work among Team Care Coordinators and Field Care Supervisors.
  • Conduct internal team audits with Care Coordinators to guarantee the delivery of the most effective and efficient service.
  • Attend regular stakeholder meetings as directed by the Care Manager.
  • Convert new client inquiries and manage staffing schedules.
  • Develop care plans and perform risk assessments to comply with relevant legal and regulatory requirements.
  • Manage a team to ensure exceptional service delivery and balanced workloads.
  • Ensure the successful operation of quality control systems and perform quality assurance visits for clients.
  • Continually review and improve processes to maintain optimal service delivery.
